Long-Term Housing & Community Services

We connect our clients with a variety of benefits, education, work opportunities, training and other building blocks to increase skills and bring to self-sufficiency. As graduates of our programs, they are prepared to meet life challenges with an independence and strength that they will need to succeed in their future.

Supported Housing Program

Our Supported Housing Program is a longer-term program designed for disabled individuals and heads of families who have experienced domestic violence. Residents are housed in their own apartments and have access to all of the same services within Transition House as our TLP residents, but this program allows for a longer transition period to independence and self-reliance.

Each individual and family in our Supported Housing Program is assessed and provided with a Goals & Service Plan. Their Goals & Service Plan evolves during their stay with us, taking into consideration all of their special needs. We partner with community agencies, organizations and service providers to put in place a permanent support system that will serve the family and/or individual well into the future.

Stabilization Services

All former residents of our Emergency Shelter, Transitional Living Program and Supported Housing Program are provided with a Stabilization Advocate. Once they become lease-holders, this advocate continues to work with them for six months or more. The stabilization program focuses on assisting clients with school registration for adults and children, access to local mental health and medical providers, and connections to community service providers.

Since we introduced this program in January of 2009, we have found it to be invaluable. All former residents who have taken advantage of this program have remained housed and have continued their goals plan.

Kent Street

For 8 years Transition House has provided support through advocacy and programming at Kent Street Apartments in Somerville. Kent Street is a diverse community that includes 40 low income units initially founded by Women’s Supported Housing and Empowerment and later supported by Transition House. Programming includes case management, crisis intervention, and programs geared to support and stimulate both children and adults.
